The underground area in Silent Hill: Townfall, the newest survival horror from No Code, Annapurna Interactive, and Konami, you reach here introduces you to the blood transfusion machine that will occupy a good chunk of your playtime. Either side of the machine holds a rack of four blood bags, each labelled with a name and a blood type, but the set-up is deliberately incomplete: the left rack is missing two bags, the right rack just one. Before you can even think about the wheel puzzle itself, you need to track all three down.
Check the blood bag racks
Have a proper look at both racks before doing anything else. The left side has two empty slots, the right side has one. Note which ones are empty, since you will be filling them in a specific order later.
Get the first bag and open the door by the telephone
The only room you can reach from here sits directly opposite the machine. Head inside and pick up the blood bag waiting for you. Carry it back out, hook it into the slot on the door near the telephone, then press Transfuse. The door opens, and the real exploring begins.
Search the first wing and survive Weight
This wing spans two floors, and nearly every room is worth checking, since the game marks bag locations on your map but rarely puts them somewhere convenient. Take your time and clear it properly.
One warning: the lower level introduces an enemy called Weight. You cannot kill it; it does not move like anything else you have fought so far, and it will find you fast, giving you only a small window to get clear. If it catches you, that is game over, so use the revival blood bag you are given for a second chance if it lands a hit. This chase was comfortably the hardest stretch of the entire game in our build.
Work through the wing room by room until you have two blood bags in hand.
Unlock the second wing and repeat
Return to the transfusion room and use one of your two bags to open access to the opposite wing. It is the same drill: search both floors, dodge Weight again, and come away with two more blood bags.
Hook the bags using Zoe's reports
Back in the main room, read Zoe's reports posted next to each empty slot and match them to the bags in your inventory. Facing the locked door, the left side takes Sarah Watson, matched to the report reading "regular pulse and head wound", and Matthew Clark, matched to "verbal confused". The right side takes George McEwan. Hook all three in, and the door will open.
Quick Tip: Don't try to fight or outlast Weight; you cannot win that way. Learn the wing's layout first if you can, so when it appears, you already know which door leads somewhere safe. The soundest strategy against Weight is distraction. The game gives you a lot of bricks and bottles in this area, so use them to distract Weight and then go the other way. Use the hiding spots as well because that's the only viable way to successfully hide from it.
