The entire transfusion puzzle in Silent Hill: Townfall is perhaps one of the toughest puzzles we encountered in video games, overall. If you have followed part one of this guide, you will have already hooked up Sarah Watson, Matthew Clark and George McEwan's blood bags and opened the reinforced door beyond the transfusion machine. What is waiting on the other side is a proper lock, a wheel wired to eight named terminals and a central hub marked Ordell, and it will not budge until you work out who can safely give blood to whom.
Recap the donor list
This new room holds a fuller donor list and a set of wall plates. Between them you get eight names and blood types: Samuel Harris (B-), Matthew Clark (B+), Sarah Watson (A+), Hannah McGregor (O-), Robert Dawson (AB-), Thomas Fergusson (O+), Derek Hill (AB+) and George McEwan (AB+). The plates spell out standard blood donor compatibility, which types can safely receive from which.
Work out who can give to whom
O- is the universal donor, so Hannah McGregor's blood is safe for everyone on the wheel. Follow the chain from there: McGregor can give to Fergusson (O+), and Fergusson can pass it on to Watson (A+). Every other combination on the dial either clashes or dead-ends, so this is the one chain that actually works.
In essence, the blood types can properly confuse you here. The real solution is which blood types can be mixed and not get spoilt for the recipient, which, in this situation, is Ordell. So if Ordell gets the mixture of O-, O+, and A+ blood types, they will stay completely safe and healthy.
Set the wheel to McGregor, Fergusson and Watson
Rotate the three rings around the dial until the only lines feeding into the Ordell hub belong to H.M., T.F. and S.W., McGregor, Fergusson and Watson. Leave the other five names alone; they are there to send you down the wrong path. Once those three lines line up, interact with the dial by navigating to the Ordell wheel (the one in the centre of the wheel) and press interact (X on the PlayStation controller), and the blood should flow straight through without reversing. This will open the hefty door ahead of you and send you out of this level.
Quick Tip: If the blood starts filling and then drains back out, do not reset everything. It usually means one ring is a single notch off, so adjust them one at a time rather than starting from scratch.
