How to solve the telephone codes puzzle
After one rather intense part of the game that involves running and gathering gear to escape from your enemy, you will find yourself inside the room with the baby-faced portrait above a fireplace and a work desk in the middle of the room. Open the bottom right drawer in the desk to pick up the document if you want, and then open the rest of the drawers and cabinets in the desk; this will trigger the phone ringing sequence.
