Given that these items all bear the same appearance, the developers at CI Games ingeniously decided to incorporate a clever twist. In some cases, these items conceal the presence of a formidable Umbral creature. When you attempt to collect such an item, it swiftly devours you, turning your entire health into a wither and relocating you to the Umbral realm.

How to pick up items guarded by Umbral Monsters
There's no visual differentiation between protected and unprotected items. To circumvent the risk of being consumed, you must apply this method to each and every item. While it may seem tedious, it's undeniably a far better alternative than being eaten and sent to Umbral unprotected.
It's worth emphasising that items dropped by enemies are free from the threat posed by Umbral Entities. You can confidently pick them up without any apprehension.
When you come across an item located in the game world, use the Umbral Lamp by aiming it at the item while holding L2, and initiate the Soulflay attack using the R2 button. In case Umbral Entity waits inside the item, this attack will extract and eliminate it, rendering the item safe for you to collect. Typically, these items contain valuable rewards.
