Baldur's Gate 3 has many secrets and while most developers are trying to showcase all their content as much as possible, Larian Studios are happy with leaving things for the players to find in subsequent playthroughs.
The Festering Cove secret area is one such example, allowing players to experience a whole new race of sentient beings they didn't encounter elsewhere.
You can find it in the Underdark, directly west of the Selunite Outpost.
The exact coordinates for the entrance, which you can see under your minimap, are X:71 and Y:-239. Even with the exact location, the entrance might be tricky to spot as it's located in the darkest part of that patch of the Underdark.
Turn towards the area that seems like it's a shadowed rock and not much else. Hover your mouse around and you will spot a highlighted cragged rock that allows you to descend into the new area.
Once you're there, you will see Kuo-Toa people worshipping what they think is a god and there are several ways you can proceed from that point onwards.
We will mostly leave it to your discretion but if you have a party member that annoys you in particular, you can sacrifice them there to get a permanent buff in return.
