Permanent stat buffs are highly sought after in Baldur's Gate 3 as they allow your characters to become unstoppable killing machines or smooth talkers the likes of which have never been seen before, depending on what you choose.
As such, they are quite attractive prospects so you will likely be willing to go through a few hoops to get them.
The big question is - are you willing to eat a part of a hag for it?
When the players go to save Mayrina, they will fight with Auntie Ethel twice. The first encounter is brief and she will attempt to escape through her fireplace and if you are powerful enough to kill her there for some reason, do not do it. Wait for her to go through in order to force the second encounter, deep in her cave.
Upon meeting Ethel in her lair, she will dangle the caged Mayrina in front of you in an attempt to distract you from the fight. Either drop rain on top of the cage to put the fire out or throw a bottle of water and that part will be done.
As you whittle Ethel's health down to under 10 per cent (12 HP on balanced mode) without killing her, she will start pleading for her life and offer power in exchange for letting her go away with Mayrina.
You can accept the deal in two ways - pass a Persuasion or Intimidation check and keep Mayrina while letting Ethel go or let her take the hostage along. In either scenario, the payment will be a small part of Ethel's scalp which you can then consume and gain one point in any attribute you choose.
Make sure Ethel is not silenced at the point where she is going down to 10 per cent HP or else she won't be able to offer you the bargain.
