Few characters in Baldur's Gate 3 are as annoying as Mayrina but she is pregnant so your conscience is likely going to push you to help her out.
Additionally, she is later involved in a quest to get a legendary weapon so that may be an additional reason to save her.
Once you've reached Ethel's lair, she will set fire to Mayrina's cage and you have a limited time until it burns off and kills the girl.
To prevent that from happening, you can simply use Shadowheart's Create or Destroy Water spell. Technically, it is not exclusive to this character but since Clerics can swap spells in and out at any point when they are not in combat, it is quite easy to bring it on her.
If you don't have anyone with Create or Destroy Water, there are numerous flasks of water that you can pick up along the way so chances are one of your characters will have it in their inventory. Simply throw it at the cage and fire will be doused.
This was the hardest part of getting Mayrina to safety and the only thing left now is to kill the hag.
Ethel has two main spells she will spam during the encounter - one creates illusory clones and the other mimics Mayrina's appearance.
Dealing with the clones is easiest if you have an Arcane Trickster (Rogue), Eldritch Knight (Fighter), Wizard or a Sorcerer in the party. These classes and subclasses have access to Magic Missile, a spell that can target multiple enemies, meaning you can target three of them at once with a level-one spell slot. Since the clones take only one hit to die, you can clear them all out in a single action. If you hit Ethel with one of the projectiles, simply finish the remaining clone off with something else.
When she copies Mayrina's appearance, you will need to be careful not to kill the girl. She yells "Get me out of this hellhole" periodically so you can recognize her by that line, just wait until she says it and then attack the other "Mayrina".
Upon reducing Ethel's health to 10 per cent or lower without killing her, she will try to make a deal by offering you a +1 stat increase in exchange for letting her go with the girl. A successful Persuasion or Intimidation check will allow Mayrina to stay while still getting the bonus. Alternatively, you can just kill the hag.
