Stone Coffin Fissure is a secret area found all the way to the south on the Realm of Shadows' map in the latest From Software DLC for Elden Ring, Shadow of the Erdtree.
This area will be very well hidden, and there's a great chance that you'll overlook it, even if you explore the place where the entrance to Stone Coffin Fissure is located.

How to access the Stone Coffin Fissure secret area
The first thing you need to do is fast-travel to the Cerulean Coast lost grace, hop on the Torrent's back, and ride all the way south to the peninsula that is located on the most southern part of the map. Once you reach there, you'll see a large hole in the ground. Go to the opposite side of the hole, and you'll notice platforms you can descend over all the way to the bottom of this hole.
Once you reach the bottom, you'll arrive to the Fissure grace. Activate it and continue following the straight path forward. The next grace, called Stone Coffin Fissure, will be close by. Activate this one as well, and press on. Soon, you'll meet familiar faces—the worm-looking enemies seen in the base game surrounding goals—only this time, they are way more aggressive and will spam magic attacks all the time, so consider yourself warned.
After passing several of those enemies and activating a couple more graces, you'll reach the Fissure Depths grace, and this one is the most important one for you. From this grace, go upstairs, climb the stone head with horns at the end of this path, and jump down to the abyss—fear not, you won't die. Once you're at the bottom, the boss fight with the Putrescent Knight will commence.
This boss is highly unpredictable, but once you get the hang of his attacks, he is not such a big deal. He rides a horse, which makes him very fast, but his attacks are highly telegraphed, and you shouldn't have much trouble dodging them. Apart from the main melee attacks, he is using frost, so be aware of these because they are very dangerous and real health-eaters. To avoid these attacks, you must jump over them, not dodge them.
Once he is defeated, activate the grace and continue forward through the tunnel to meet St. Trina NPC. This NPC is very strange because she doesn't speak, but instead, she will give you the opportunity to imbibe her nectar. If you do so, you'll die, but it isn't a big deal because you'll be respawned on the latest grace, and all your runes will be recoverable.
If you're wandering, should you imbibe the nectar? Yes, you should, and not once, but four times. After the fourth death, St. Trina will talk to you, giving away some useful information about Miquella. Keep imbibing nectar until she doesn't have anything new to offer—she will say that you must kill Miquella.
Now, what's interesting about this quest is the presence of Thiollier in Deep Purple Garden. Depending on your choices when speaking with him earlier, he might or might not be there. If he is there, you'll be able to continue the quest. If he isn't, then the quest is broken for you. If he is present, speak with him, and he will call you a liar.
Rest at the grace and he will invade your world. Defeat him, and you'll get the St. Trina Smile talisman. However, the quest isn't done yet. After defeating the main boss (easier said than done), return to Deep Purple Garden, and you'll find St. Trina's dead body with the St. Trina Blossom item next to it.
