In Baldur's Gate 3, Art Cullagh is one of the first Flaming Fists who tangled with the Sharrans led by Ketheric Thorm a century ago.
When you arrive at the Last Light Inn, the man is comatose and keeps rambling about Thaniel, with the only clue about his past being the House of Healing.
House of Healing is a little west of the Last Light Inn so before you do that, we would highly suggest getting the shadow curse protection from Isobel and completing the subsequent encounter. Make sure Isobel doesn't get kidnapped or the quest to save Art will fail.
Once you're done with the would-be kidnappers, proceed to the House of Healing, which has undead nurses roaming the place, as well as the surgeon Malus Thorm.
He is surrounded by four other nurses, who will help him during the boss fight. It is recommended that you bring someone with high Charisma in order to persuade Malus to either fight you without the nurses or to let him get killed by them, if you don't feel comfortable fighting him.
When he goes down, loot the boss' corpse to get a unique lute, which has an inscription suggesting it belonged to Art Cullagh. Bring it back to the Last Light Inn and engage in conversation with the comatose Fist once again.
You will now have the option to play the lute instead of the regular lines. Choose it and Art will spring to life. He will then let you know who Thaniel is and give you a clue on how to lift the shadow curse.
