To use the Bright Red Apple, your initial step is to reach the Estella Opera House level. Once you're at the Stargazer's location, enter the Opera House and ascend the staircase. Choose the right-hand staircase, and then continue along the path by climbing the stairs.
At this point, you'll encounter several puppets and a more menacing spider-like puppet. After disposing of these foes, proceed along the only available route, which will lead you to a balcony with a swinging chandelier below. From there, make two consecutive left turns, open the door, and you'll find yourself in the next room, where you'll meet Adelina, an opera singer who has lost her voice due to illness.
Following your initial conversation with Adelina, restart the dialogue, and she will express her longing for a real meal after a prolonged period without one. You will then be presented with the option to either offer her an apple or refrain from doing so. Choosing to give her an apple showcases your empathy and unquestionably boosts your humanity points significantly.
Regrettably, apart from the humanity points, there won't be any additional rewards, with the exception of Adeline's confession.
