One of the puzzles in Pathfinder: Wrath of the Righteous that may seem bugged resides in Conundrum Unsolved location but here is the solution and reasons why it's not.
Conundrum Unsolved is a puzzle that stretches several chapters, which is the leading reason why Pathfinder: Wrath of the Righteous players might think it's bugged.
The first reason why you might think it's bugged could be the missing "loot" from the prompts on the floor. You don't actually loot anything there, you are supposed to put the corresponding slabs in their proper slots. The first batch of slabs is found in Kenabres before you even leave the city, in the location called Topaz Solutions. This location will be drawn on your map after you talk with Anevia in Defender's Heart and ask her about suspicious locations.
After that, you will need to loot four more slabs in a location outside the city, after you've liberated it from the demon threat. These can be found in the Underground Hideout, located just across the river from Leper's Smile.
Conundrum Unsolved puzzle solution
With all the slabs in your inventory, you can finally go to Conundrum Unsolved and solve the puzzle. Align the outer slabs in the only way they can be placed but keep in mind that the slabs that go into the square itself can be rotated, depending on which "Loot" icon you click.
Also, you will need to check the symbols on the slabs in your inventory in order to place them correctly. Once all that is out of the way, align them like it's shown in the picture below.
After that, a door will open with some loot behind it so don't forget to pick the items up after you're done conversing with the ghost. It will point you to a location in the west, one that will not appear on your map. This is not bugged either - you will only be able to find the location after you're done with Drezen. Good luck!
